The optimum management protocol for NHAP remains undetermined. The majority of episodes of NHAP are managed in the nursing home setting, but the factors that influence hospitalization for initial treatment remain nebulous, though non-clinical and financial issues may be important. Clinical pathways for management of NHAP have been published, but they have several limitations that may reduce their utilization. The uncertainty regarding the choice of antibiotic therapy for NHAP relates to the inability to identify the etiologic agent in most cases and the lack of clinical trials. At present, a fluoroquinolone gatifloxacin, levofloxacin, or moxifloxacin ; appears to be the most reasonable empiric regimen for NHAP treated in the nursing home or hospital because of its dosing schedule and availability of an oral formulation, activity against most common etiologic agents of NHAP, and excellent safety profile overall. However, there is the caveat that recent antibiotic therapy prior to pneumonia or history of colonization with resistant organisms may alter that choice in some cases. For the small percentage 2% ; of cases of NHAP with severe respiratory failure that require mechanical ventilation, gram-negative bacilli including P. aeruginosa and S. aureus appear to be the most common pathogens and empiric therapy should be directed to these pathogens. Duration of therapy has not been studied for NHAP, but most episodes should be successfully treated with seven days of therapy. Future studies of NHAP should focus on applying molecular methods to identify causative agents, using practical clinical findings to define severity in the nursing home setting, establishing evidence-based criteria for hospitalization decisions, determining the optimum treatment regimen and duration of therapy, distinguishing between pneumonitis and pneumonia, and further evaluation of withholding antibiotic therapy in some residents with advanced dementia and pneumonia.
