Results : All the 252 patients having diffuse or nodular goiter with euthyroid function had a TPR value below 2.52 mean TPR + 2 SD normal volunteers ; .Out of 578 patients having Hyperthyroidism 515 had a TPR value more than 2.52 and 63 patients had TPR less than 2.52. T3 and T4 values were high in 512 patients and normal in 66 cases. In 18 patients who were clinically hypothyroid 12 had low TPR 2.52 ; whereas 6 showed TPR more than 2.52. Five of them later found to have iodine avidity and one dyshormonogenesis. In 22 patients clinically suspected to have Thyroiditis, 14 had low TPR whereas 8 revealed higher values of TPR. All these patients had also high T3 and T4 values and in follow up found to be suffering from mild hyperthyroidism. The sensitivity and specificity of TPR has been calculated to be 93.7 % and 88.57 % respectively. Recombinant human activated protein C drotrecogin alfa [activated] ; has been shown to have anti-inflammatory, anti-thrombotic and anti-apoptotic properties. In this study we assessed the hypothesis that activated protein C APC ; may have a cardioprotective effect via preventing apoptosis in a rat model of myocardial I R. Methods: Thirty male Sprague-Dawley rats were anesthetized, instrumented for hemodynamic measurements and ventilated mechanically. Twenty rats were subjected to 20 minutes of left anterior descending coronary artery occlusion and 120 minutes of reperfusion. They were randomly assigned to receive intravenous Ringer lactate vehicle ; or APC 2 mg kg hr ; 10 minutes after occlusion and during reperfusion. The other 10 rats were sham-operated. At the end of the reperfusion period serum samples were obtained for CPK and TNF-a evaluation. Apoptosis was measured quantitatively by the terminal deoxynucleotide transferasemediated dUTP nick-end labeling TUNEL ; method. Results: Baseline mean arterial pressures and heart rates were similar among all groups. During reperfusion mean arterial pressures were significantly higher in APC treated rats compared with the control group 70.1 15.9 and 54.6 11.9 mmHg, respectively, p 0.043 ; . Serum CPK and TNF-a values and percentage of TUNEL positive myocyte nuclei demonstrated negligible myocardial injury in sham-operated controls. There was no difference between APC treated and untreated animals with respects to CPK and TNF-a levels. Number of apoptotic cells was significantly reduced from 47.7% to 24.8% with APC administration p 0.008 ; . Conclusions: Our data demonstrate that treatment with APC significantly improves hemodynamics after I R and reduces I R induced myocardial apoptosis in rats. However, further studies are required to clarify the cardioprotective role of APC in myocardial I R.
